Bio-Identical Hormone Replacement Therapy

As we age our hormone levels decline. Both men and women experience hormonal declines. For women, the decline is most noted at menopause. Men, on the other hand may have a much more insidious decline and have their own version of menopause, called andropause. One perspective is that the decline in hormones is “normal”. Well, dying is “natural” too but we certainly want to delay that as long as possible! I’ve had several patients tell me that their doctor says that their hormones are “normal” or “fine” for their age.

You may have joint pain, low energy, and a declining libido which could also be considered “normal” for your age. Go enjoy it, right!? However, NORMAL is not OPTIMAL and there are many treatments we can do to optimize your hormone levels and prevent and improve many of the health problems associated with aging. Getting old is normal, feeling old is optional.

Sexual Enhancement For Women

Interestingly, women are more likely to have sexual problems, like low libido (low sex drive), but they are rarely told about the many options that can make sex more enjoyable.

“While research suggests that sexual dysfunction is common (43% of women and 31% of men report some degree of difficulty), it is a topic that many people are hesitant or embarrassed to discuss. Fortunately, most cases of sexual dysfunction are treatable, so it is important to share your concerns with your partner and doctor.” WebMD

WebMD goes on to say that the causes can either be physical or psychological. This is correct, but often times the physical is effecting the psychological since they are connected (of course). Hormones effect the brain and this is well documented (ie PMS is usually a progesterone deficiency). A very good book that discusses hormones and the female brain is called “The Female Brain,” by Louann Brizendine, MD. Dr. Brizendine discusses in detail how hormones can effect libido (sex drive), mood, and relationships.

For example, oxytocin is a hormone that your brain produces when you have an orgasm, make eye contact, or touch another person. Some people don’t make enough of this hormone and can have an oxytocin deficiency. Low oxytocin can cause you to have low libido but also make it difficult to have an orgasm during sex further reducing sexual desire and enjoyment. Luckily, with a little oxytocin under the tongue (sublingual) you can greatly enhance your libido and also have a much better chance of having multiple orgasms during sex.

Low Libido Treatments For Women

Low libido in women can often be improved by balancing hormones such as DHEA and testosterone. DHEA is one of the most plentiful hormones your body makes but this hormone declines with age. In a recent study they researched the use of daily DHEA in older women:

The secretion and the blood levels of the adrenal steroid dehydroepiandrosterone (DHEA) and its sulfate ester (DHEAS) decrease profoundly with age, and the question is posed whether administration of the steroid to compensate for the decline counteracts defects associated with aging. The commercial availability of DHEA outside the regular pharmaceutical–medical network in the United States creates a real public health problem that may be resolved only by appropriate long-term clinical trials in elderly men and women. Two hundred and eighty healthy individuals (women and men 60–79 years old) were given DHEA, 50 mg, or placebo, orally, daily for a year in a double-blind, placebo-controlled study.

The authors found many benefits from a daily DHEA dose:

“No potentially harmful accumulation of DHEAS and active steroids was recorded. Besides the reestablishment of a “young” concentration of DHEAS, a small increase of testosterone and estradiol was noted, particularly in women, and may be involved in the significantly demonstrated physiological–clinical manifestations here reported. Bone turnover improved selectively in women >70 years old, as assessed by the dual-energy x-ray absorptiometry (DEXA) technique and the decrease of osteoclastic activity. A significant increase in most libido parameters was also found in these older women.

Improvement of the skin status was observed, particularly in women, in terms of hydration, epidermal thickness, sebum production, and pigmentation. A number of biological indices confirmed the lack of harmful consequences of this 50 mg/day DHEA administration over one year, also indicating that this kind of replacement therapy normalized some effects of aging, but does not create “supermen/women” (doping).”1

Testosterone levels in women also decline with age and this can have a huge impact on libido (sex drive).2 While women have about one tenth the testosterone level of men, the decline that occurs with aging can create problems in intimate relationships. When taking hormones we always consider the other hormones because balance is the key to optimal well being.

Some women report that when they apply testosterone cream to the clitoris they enjoy sex even more. This could be due to the vasodilatory effects of testosterone. Nitric oxide (NO) is a signalling molecule for vasodilation. Because testosterone improves NO signalling this makes sense. Taking the amino acid L-citrulline can also help improve blood flow if taken by mouth daily. L-citrulline converts to nitric oxide in your body’s tissues. In excess, testosterone cream can cause the clitoris to become enlarged. This is rarely a problem in older when due to the decrease in the size of the clitoris that occurs with aging.

Dyspareunia (Pain With Intercourse) Treatments For Post-Menopausal Women

While almost half of women over 50 years of age suffer from pain with intercourse due to vaginal atrophy, only 25% seek treatment. For most women, estrogen applied vaginally will cure vaginal atrophy and when balanced with progesterone and other hormones potential side effects are minimal.3 The use of bio-identical hormones makes the most sense since they are identical to the hormones your body made when you were 25.

Mood Enhancement

Many things decline with age and this often can include your mood. Low mood and mild depression are often symptoms of hormone deficiencies or imbalances. It is pretty rare to see an older person walking down the street with a big smile. Feeling happy comes from within and their is nothing more inner that hormones because they effect your DNA expression. When your body is in balance you will feel more at ease.
